STEP1.

You need to verify your identity if you have forgotten your login ID or registered e-mail address.
Please send an e-mail with items (1) and (2) to the address below.
The Alumni Office will contact you shortly.

• (2)Main text：Name (at time of enrollment)

STEP2.

Once you have received an e-mail from the Alumni Office with the subject line 'Identity verification request accepted',
please register by filling in only the required fields on the Alumni Registration Form.

STEP3.

We will verify your identity using the details provided in the form.
If verification is successful, your login ID and password will be sent to the e-mail address provided in the form above.
Please note that if verification is unsuccessful, we will take further steps to confirm your identity.
